First Assist Urgent Care

Welcome to FIRST ASSIST URGENT CARE.  We are glad you have chosen us to care for your illness or injury. You can be confident that our staff will work hard to make you feel as comfortable as possible during the time you are with us. We want you to feel good about your experience and the care provided at First Assist Urgent Care. Please take time to review this information. This will answer many questions, help make your visit go smoothly, and improve your satisfaction with our services.

We are convinced that the best patient-provider relationship is a result of quality professional service, compassion, and understanding. We strive to provide this to our patients.

Insurance and Fees

Please bring your insurance card with you for each visit. We will make a copy of it when necessary. It is your responsibility to know what your insurance covers. We will need your assistance to ensure that your insurance is filed appropriately. Thank you for assisting us in this matter.

The charges for each patient are based on the professional services provided as well as laboratory and other diagnostic tests necessary for the individual patient or problem. The professional services component of your charges are determined by but are not limited to the following: the amount of time spent with the provider, the complexity and severity of your problem(s), the extent of your examination, and the number and type of diagnostic tests ordered.

Therefore, only general information about an estimate of charges can be given before the history and physical examination have been done.

Due to the fact that there are multiple insurance plans, many of which frequently change their policies, we are often unable to determine what services are covered under your specific insurance plan. This often includes what medicines and tests are covered. Please review your current insurance coverage policies before your office visits.

Information regarding your office visit must be accurately documented in your medical record and cannot be altered. Please do not ask us to alter your diagnosis, medical record, or description of services rendered for insurance payment purposes. Inappropriate alteration of medical records can be considered a fraudulent and punishable offense by the Office of Inspector General.

We know that healthcare costs are expensive and we will assist you in filing insurance claims on your behalf. Thank you for assisting us in this matter.

Your co-pay portion, deductible, and non-covered services must be paid at time of service.
